SARS-CoV-2/COVID-19: CLIENT NOTICE
A central goal of Acumen Assessments is to provide a safe environment for our clients and staff and to advance the safety of our community. We would like to offer the below information for educational purposes as well as to state our protocols, suggestions, and requirements for working together at this time.

Acumen Assessments wants you to be aware of the risks of contracting COVID-19 before, during, and after your evaluation. SARS-CoV-2 is a highly contagious virus that can lead to serious illness and even death. Early data suggests that older individuals with certain comorbidities (e.g. hypertension, obesity, diabetes, respiratory or heart disease, etc.) may be at greater risk. You could contract COVID-19 from a variety of sources.

The COVID-19 disease has a long incubation period. You, your patients, other clients, and our staff may have the virus that causes COVID-19 and not show symptoms, yet still be highly contagious. Determining who is infected by the virus is challenging and complicated due to the limited availability for virus testing and the accuracy of testing. All staff wear masks in the office and in public places and pay close attention to their health and the health of those around them. We are all vaccinated with the Moderna vaccine and have had the booster.

In times of social isolation, the very act of leaving home and coming to our facility carries some inherent risk of SARS-CoV-2 exposure, and then COVID-19. Acumen Assessments is working to provide a safe and sanitary experience as currently possible by following CDC requirements.

The simplest, most scientifically-proven prevention methods to date include:

Vaccination (Moderna or Pfizer and the booster) - This is required. If you received the J&J vaccine, we ask that you get an mRNA booster or what the CDC is calling the mix-and-match policy

Wearing a surgical mask, N95, or KN95 (we encourage the latter based on current recommendations)

Maintain social distancing, if possible

Minimizing interaction with other people except as necessary

Get take-out food rather than eating in restaurants

Avoid any close contact with people who are sick with COVID-19 symptoms (cough, fever, body aches, and general fatigue)

Frequent hand washing or sanitizing for at least 20 seconds each time, especially before and after eating or touching the face or nose, putting on and taking off face masks or coverings.

Regular and frequent cleaning of high-use surfaces
We will ask you to acknowledge reviewing this information fully, and confirm that you understand its contents. We will ask that you accept there is an increased risk of contracting the COVID-19 disease if you visit or receive services outside of your home. We will ask that you understand and accept the additional risk of contracting COVID-19 from contact in our facility. We will also ask you to acknowledge that you could contract the COVID-19 disease from outside this facility and unrelated to your visit with us. We will ask you to agree to abide by the COVID-19 prevention measures listed above. We will ask you to agree to ask the staff, your primary doctor, and dwelling partners, and those who come in near contact with you to do the same.
